Chasing down paper forms and signatures is nobody's idea of a good time. With SignUpGenius Forms and Waivers, you can collect information and signatures from your participants without printing a single page. Here's how to build and publish your form from start to finish.
Make sure you're signed in to your SignUpGenius account before you begin.
If this is your first time creating a payment page, you'll see a setup screen with a few options to get started. We'll also ask for some information we need so you can collect payments. You don't have to complete this part right away, but you will need to before you can withdraw funds.
If you've created a payment page before, click Create from your Dashboard and select Forms or Waivers.
Add your form details
Enter your title, description, and a contact name for questions. If your form is tied to an in-person event, add the date, time, and location.
From here, you can also:
- Customize the confirmation email participants receive after submitting
- Schedule your form to open and close at a specific date and time
- Restrict access with a code you distribute to participants
- Add FAQs to answer common questions upfront
Click Save.
Build your form
Drag basic fields onto the form to collect standard information, like name and address. Need something more specific? Add a custom question for anything not covered by the default options.
To turn your form into a waiver, add a waiver text block and a signature line — participants will need to sign before submitting. You can also decide whether to require the form or waiver of anyone checking out.
If you need to collect different sets of information, click Save and Add at the bottom of the builder to add another form. Otherwise, click Save.
Publish and share
Give your form one last look, then click Publish to send it out into the world. Heads up: publishing doesn't notify anyone on its own, so it's on you to spread the word. Get your form in front of people by:
- Sending or posting a direct link
- Adding a custom button to your website
- Generating a QR code for printed materials
- Creating a text code participants can send for a link
